My activities on August 23 2022 | Story of a young patient with lipid disorder

Hi there! Assalamu Alaikum. How are you? Hope that you are well. I am fine as well, Alhamdulillah. Here are my activities on the 23rd of August 2022. It was Tuesday. It is my Actifit report number 800! In this post, I have included the following things:

Story of a patient with dyslipidemia

A 32-year-old patient visited me yesterday. He is working in one of the top petroleum development companies in Oman and the treatment costs of the employees are covered by the company. Some employees are taking advantage of it and coming to us for some unnecessary investigations. This is not an insurance company. Hence, there is no hassle in getting approval from the company (unlike insurance companies; most of the cases need pre-approval before doing a test) for the investigations. The patient did lipid profile, HbA1c and CBC tests on the 31st of July 2022. At that time his lipid profile and HbA1C results were as follow:

HbA1C: 5.5% (the normal value is < 6%). It is a test done on a diabetic person to assess the average blood sugar level in the previous 3 months.

Lipid profile:

  • Total cholesterol: 308 mg/dl (Normal value: < 200 mg/dl)
  • Triglyceride: 75 mg/dl (Normal value: 50-165 mg/dl)
  • HDL cholesterol: 31 mg/dl (Normal value: 35-75 mg/dl)
  • LDL cholesterol: 262 mg/dl (Normal value: 70-105 mg/dl)
  • VLDL cholesterol: 15 mg/dl (Normal value: 10-35 mg/dl)

So, even though he wasn’t a diabetic patient, he did the HbA1C test which was normal. However, he had abnormal lipid results. At that time he didn’t want to start medicine for a lipid disorder. So he wanted to try non-drug management first.

Yesterday, when he visited and asked me to repeat the investigations, I was hesitant at first to do it. It wasn’t even a month since his last tests. We are doing HbA1C and Lipid profile tests usually every three months or so. The values aren’t changing over days or weeks. It takes months to see the effect of the treatments. Anyway, as he insisted, I ordered the tests.

His new tests results were as follows:

HbA1C: 5%

Lipid profile:

  • Total cholesterol: 417 mg/dl
  • Triglyceride: 75 mg/dl
  • HDL cholesterol: 42 mg/dl
  • LDL cholesterol: 366 mg/dl
  • VLDL cholesterol: 15 mg/dl

His HbA1C was almost the same. So, it wasn’t justifiable this time again. However, two components (Total cholesterol and LDL) of his lipid profile that were high before, were raised to higher levels! Regarding lipid profile, looks like it was a good decision to repeat the test! His non-drug management failed to control his lipids level. He now needs pharmacological management alongside non-drug management.

Sometimes, the theory and the reality are quite different!

Chamber activities

On Tuesday, in my chamber, I consulted 25 patients. There was no emergency case, Alhamdulillah. And, in the visa medical section, 35 people came for the check-up.

Workout

In the afternoon, after lunch, I walked on the treadmill for around half an hour.
